About Our Program
Pre-Admission Screening and Resident Review (PASRR) is a federally required screening of any individual prior to admission into a Medicaid-certified nursing facility, regardless of the source of payment. Individuals with mental illness, intellectual disability, or a related condition have special protections under state and federal law to ensure that long-term services and supports are provided in the right setting to meet their individual needs. Our Acentra Health PASRR team is honored to have been selected as one of Washington’s new PASRR vendors! Our team will support the State of Washington’s PASRR program by providing comprehensive Level II Evaluations for individuals with suspected or confirmed mental illness.
Our team of Mental Health Professionals, licensed in the state of Washington, utilizes a multi-disciplinary approach consisting of highly skilled and trained professionals, including master's level psychologists, nurses, social workers, and other mental health professionals. We work together to identify individuals with possible mental illness and provide comprehensive evaluation prior to admission to nursing facilities or upon significant change in condition for residents within nursing facility settings. PASRR Level II evaluations are person-centered and assess nursing facility level of service needs and additional care needs, such as Specialized Services for Behavioral Health, to ensure individuals receive needed services in the least restrictive setting possible.
As of July 1, 2024, Acentra’s PASRR Evaluators will provide face-to-face Level II evaluations for individuals with possible or confirmed mental illness for the following counties:
- King
- Kitsap
- Grays Harbor
- Jefferson
- Lewis
- Clallam
- Yakima
- Klickitat
If you work at a nursing home or hospital in one of the above counties, please submit your Level II referral directly to us using the contact information below. Please provide us with a copy of the Level I and all relevant supporting documentation, such as a current history and physical, medication list, and recent medical notes (including nursing and psychiatric if available). We will immediately reach out to schedule the PASRR Level II evaluation.

We will respond to afterhours inquiries on the following business day. Please note that our clinicians are often completing evaluations during the weekends and are regularly checking messages in case of emergencies.
